ABSTRACT

ABSTRACT Purpose: To describe the implementation pro cess and the preliminary results of a surveillance system for healthcare-associated endophthalmitis. Methods: This is a case study of the implementation of a surveillance system for healthcare-associated endophthalmitis. The system for healthcare-associated endophthalmitis is a structured system that enables surveillance of cases of healthcare-associated endophthalmitis after intraocular procedures, developed and coordinated by the Division of Hospital Infection at the State Health Department, São Paulo, Brazil. The implementation process included a pilot phase, followed by a scaling-up phase. Data were reported monthly to the Division of Hospital Infection by participating healthcare facilities that performed intraocular procedures in the state of São Paulo, Brazil, from September 2017 to December 2019. Results: Among the 1,483 eligible healthcare facilities, 175 engaged in the study (participation rate of 11.8%), reporting 222,728 intraocular procedures performed, of which 164,207 were cataract surgery and 58,521 were intravitreal injections. The overall incidence rate of endophthalmitis was reported to be 0.05% (n=105; 80 cases after cataract surgery and 25 cases after intravitreal injections). The incidence rates for healthcare facilities ranged from 0.02% to 4.55%. Most cases were caused by gram-positive bacteria, mainly Staphylococcus spp. In 36 (46.2%) of the cases, there was no bacterial growth; no sample was collected in 28 (26.7%) cases. This system for healthcare-associated endophthalmitis enabled the identification of an outbreak of four cases of endophthalmitis after intravitreal injections. Conclusion: The system for healthcare-associated endophthalmitis proved to be operationally viable and efficient for monitoring cases of endophthalmitis at the state level.


RESUMO Objetivo: Descrever o processo de implementação e os resultados preliminares de um sistema de vigilância epidemiológica para endoftalmites associada à assistência à saúde. Métodos: Trata-se de um estudo de caso de implementação de um sistema de vigilância epidemiológica para endoftalmites. O sistema de vigilância epidemiológica para endoftalmites é um sistema estruturado que possibilita a vigilância de casos de endoftalmite associados à assistência à saúde após procedimentos oftalmológicos invasivos, desenvolvido e coordenado pela Divisão de Infecção Hospitalar da Secretaria de Estado da Saúde, São Paulo, Brasil. O processo de implementação incluiu uma fase piloto, seguida pela fase de expansão. Os dados foram enviados mensalmente à Divisão de Infecção Hospitalar pelos estabelecimentos de saúde participantes que realizaram procedimentos oftalmológicos no estado de São Paulo, Brasil no período de setembro de 2017 a dezembro de 2019. Resultados: Entre os 1.483 estabelecimentos de saúde elegíveis, 175 participaram do estudo (taxa de adesão de 11,8%), relatando 222.728 procedimentos oftalmológicos realizados, sendo 164.207 cirurgias de catarata e 58.521 injeções intravítreas. A taxa de incidência global de endoftalmite relatada foi de 0,05% (n=105; 80 casos após cirurgia de catarata e 25 casos após injeção intravítrea). As taxas de incidência entre os estabelecimentos de saúde variaram de 0,02% a 4,55%. A maioria dos casos foi causada por bactérias gram-positivas, principalmente Staphylococcus spp. Em 36 (46,2%) casos não houve crescimento bacteriano; nenhuma amostra foi coletada em 28 (26,7%) casos. O sistema de vigilância epidemiológica para endoftalmites possibilitou a identificação de um surto de quatro casos de endoftalmite após injeção intravítrea. Conclusão: O sistema de vigilância epidemiológica para endoftalmites mostrou-se operacionalmente viável e eficiente para o monitoramento de casos de endoftalmite em nível estadual.

Purpose: To assess the current trends in use of perioperative antibiotic prophylaxis, especially intracameral antibiotics (ICAB), for cataract surgery in India. Methods: This was an E?survey using a previously validated questionnaire carried out by the All India Ophthalmological Society (AIOS) in August 2022. An E?mail invitation to complete an online 40?point survey was sent to all members of the AIOS using a digital E?mail service (Survey Monkey) and social media platforms. Results: Out of 1804 total respondents, 58% (n = 1022) reported using routine ICAB prophylaxis. Of those using ICAB, 89% (n = 909) reported using it for all cases, whereas the rest used it for specific indications such as post?PC rent (n = 43), one eyed patient (n = 19), and high?risk cases (n = 50). Commercially available moxifloxacin was by far the commonest agent used for ICAB (n = 686, 67%). Eighty respondents (8%) reported noticing occasional toxic reactions and nontoxic corneal edema (n = 64, 6%) with ICAB. Surgeons with >10 years� experience especially practicing in medical colleges used fewer ICAB (OR = 0.52, 95%CI = 0.38 � 0.72, P < 0,001) compared to younger surgeons (<5?year experience) in solo clinics. The commonest reason for nonadoption of IC antibiotics was 搖nconvinced of the need to use it in my setting� (n = 296, 52% of those who answered this question). Conclusion: IC antibiotic prophylaxis for cataract surgery has significantly increased in India from 40% in 2017 to 58% in 2022. Commercially available moxifloxacin was preferred by the majority users. Intraocular inflammation may occur occasionally and needs safer formulations to avoid this

ABSTRACT Purpose: To present the results of a retrospective study regarding the clinical and economic impact of intracameral cefuroxime administration to prevent endophthalmitis during cataract surgery in a referral hospital. Methods: This study included 16,902 eyes from patients who had undergone cataract surgery between 2013 and 2017. From May 2014 onwards, all patients received routine intracameral injections of 1 mg cefuroxime (10 mg/1 mL) after phacoemulsification. The prophylactic efficacy was evaluated using the relative risk ratio, whereas the economic impact was evaluated using number needed to treat to avoid endophthalmitis. Results: Before introducing cefuroxime, 3,407 cataract surgeries were performed using the phacoemulsification technique, and 7 post-operatory cases of endophthalmitis occurred (0.2% incidence). After introducing the cefuroxime protocol, 13,495 surgeries were performed, and 4 endophthalmitis cases were registered (0.03% incidence). Cefuroxime was identified as a protective factor against the development of endophthalmitis [risk ratio = 14%, p=0.002, 95% confidence interval (CI) 95%, 4%-49%], with an economic impact of number needed to treat = 568. The potential savings with cefuroxime was approximately US $2,334.36 for every 568 patients treated. Conclusion: The incidence of endophthalmitis decreased by 86% (risk ratio = 14%, p=0.002, 95% CI, 4%-49%) after introducing intracameral cefuroxime prophylaxis at the study hospital. The results presented herein provide strong evidence for the use of cefuroxime in endophthalmitis prophylaxis after phacoemulsification surgeries, outperforming the alter­native by providing both economic and clinical benefits.


RESUMO Objetivo: Apresentar os resultados de um estudo retrospectivo sobre o impacto clínico e econômico da administração de cefuroxima intracameral para prevenir endoftalmite nas cirurgias de catarata em um hospital de referência. Métodos: Este estudo incluiu 16.902 olhos de pacientes submetidos à cirurgia de catarata entre 2013 e 2017. A partir de maio de 2014, todos os pacientes receberam rotineiramente uma injeção intracameral de 1mg de cefuroxima (10mg/1mL) ao final da cirurgia de facoemulsificação. A eficácia da profilaxia foi avaliada usando o risco relativo e o impacto econômico foi avaliado com o número necessário para tratar para se evitar um caso de endoftalmite. Resultados: Antes da introdução do protocolo da cefuroxima, foram realizadas 3.407 cirurgias de catarata por facoemulsificação e ocorreram 7 casos de endoftalmite pós-operatória (incidência de 0,2%). Após a introdução do protocolo da cefuroxima, foram realizadas 13.495 cirurgias e registrados 4 casos de endoftalmite (incidência de 0,03%). A cefuroxima foi um fator de proteção no desenvolvimento de endoftalmite (risco relativo = 14%, p=0,002, Intervalo de Confiança de 95% [IC 95%], 4% - 49%) e o impacto econômico do número necessário para tratar = 568. A economia potencial com a cefuroxima foi de aproximadamente US$ 2.334,36 para cada 568 pacientes tratados. Conclusão: A incidência de endoftalmite diminuiu 86% (risco relativo = 14%, p=0,002, IC 95% 4% - 49%) desde a introdução da profilaxia com cefuroxima intracameral no hospital do estudo. Os resultados apresentados mostram forte evidência para o uso da cefuroxima na profilaxia da endoftalmite após cirurgias de facoemulsificação, por proporcionar economia de custos e benefício clínico.

ABSTRACT

This case report describes three eyes of two patients, who were diagnosed to have endogenous fungal endophthalmitis post coronavirus disease 2019 (COVID-19) infection. Both patients underwent vitrectomy with intravitreal anti-fungal injection. Intra-ocular samples confirmed the fungal etiology by conventional microbiological investigations and polymerase chain reaction in both cases. The patients were treated with multiple intravitreal and oral anti-fungal agents; however, vision could not be salvaged.

ABSTRACT Staphylococcus hominis (S. hominis) is a coagulase-negative Staphylococci and an infrequent cause of endophthalmitis. Due to its ability to produce biofilm, especially in diabetic patients, strains may acquire antibiotic resistance. We present two cases of S. hominis endophthalmitis, one with acute endophthalmitis after intravitreal bevacizumab injection and one with chronic endophthalmitis following undiagnosed penetrating ocular trauma. Although there are only four published S. hominis endophthalmitis cases in the literature, to the best of our knowledge, there has been no previously published case after intravitreal bevacizumab.


RESUMO Staphylococcus hominis (S. hominis) é um estafilococo coagulase-negativo e uma causa pouco frequente de endoftalmite. Devido à sua capacidade de produzir biofilme, especialmente em pacientes diabéticos, cepas dessa bactéria podem adquirir resistência a antibióticos. Este relato apresenta dois casos de endoftalmite por S. hominis: um de endoftalmite aguda após injeção intravítrea de bevacizumabe e outro de endoftalmite crônica após trauma ocular penetrante não diagnosticado. Embora existam apenas quatro casos de endoftalmite por S. hominis publicados na literatura, até onde sabemos não houve nenhum caso publicado anteriormente após bevacizumabe intravítreo.

Introducción: La endoftalmitis posquirúrgica es la complicación más temida de la cirugía de catarata. Resulta potencialmente devastadora, puede amenazar seriamente la visión y tiene una incidencia estimada de entre 0,02 y 0,71 por ciento. Objetivo: Determinar la incidencia de endoftalmitis poscirugía de catarata y su comportamiento clínico. Métodos: Se realizó un estudio observacional, descriptivo y de corte transversal. Se revisaron 13 850 cirugías consecutivas de catarata realizadas en el Centro Oftalmológico del Hospital Universitario Clínico Quirúrgico "Arnaldo Milián Castro" de Villa Clara, Cuba. Resultados: La incidencia de endoftalmitis poscirugía de catarata en esta serie fue de 0,17 por ciento (IC 95 por ciento: 0,10-0,24 por ciento); 0,18 por ciento (IC 95 por ciento 0,11-0,25 por ciento) para extracción extracapsular del cristalino y sin incidencia en la facoemulsificación. La forma de presentación aguda fue más frecuente que la crónica; 0,13 por ciento (IC 95 por ciento: 0,07-0,19 por ciento) y 0,04 por ciento (IC 95 por ciento: 0,01-0,07 por ciento), respectivamente. Los hombres fueron más afectados que las mujeres y la edad media fue de 71,8 años. La forma aguda se presentó con una media de 5,1 días entre la cirugía y el inicio de los síntomas y la crónica con una media de 21,2 semanas. El 39,1 por ciento de los pacientes tuvo agudeza visual de percepción luminosa al momento del diagnóstico. Se reportó un crecimiento bacteriano en el 44,7 por ciento de las muestras, con una positividad en humor acuoso y vítreo del 42,1 por ciento y 47,4 por ciento, respectivamente. El Staphylococcus coagulasa negativo fue el germen más frecuente. Los antibióticos intravítreos más vitrectomía plana precoz fue la conducta terapéutica más empleada. Conclusiones: La incidencia de endoftalmitis poscirugía de catarata en esta serie está en el rango de lo reportado a nivel mundial. Se presenta comúnmente de forma aguda y con pobre agudeza visual. El agente etiológico aislado con más frecuencia fue el Staphylococcus coagulasa negativa(AU)


Introduction: Postoperative endophthalmitis is the most feared complication of cataract surgery. It is potentially devastating, can seriously threaten vision and has an estimated incidence of between 0.02 and 0.71 percent. Objective: To determine the incidence of endophthalmitis after cataract surgery and its clinical behavior. Methods: An observational, descriptive, cross-sectional, descriptive study was performed. Thirteen 850 consecutive cataract surgeries performed at the Ophthalmologic Center of the Clinical Surgical University Hospital "Arnaldo Milián Castro" of Villa Clara were reviewed. Results: The incidence of endophthalmitis after cataract surgery in this series was 0.17 percent (95 percent CI: 0.10-0.24 percent); 0.18 percent (95 percent CI: 0.11-0.25 percent) for extracapsular extraction of the crystalline lens and no incidence in phacoemulsification. The acute form of presentation was more frequent than the chronic form; 0.13 percent (95 percent CI: 0.07-0.19 percent) and 0.04 percent (95 percent CI: 0.01-0.07 percent), respectively. Males were more affected than females and the mean age was 71.8 years. The acute form occurred with a mean of 5.1 days between surgery and symptom onset and the chronic form with a mean of 21.2 weeks. Thirty-nine.1 percent of patients had visual acuity of light perception at the time of diagnosis. Bacterial growth was reported in 44.7 percent of the specimens, with positivity in aqueous and vitreous humor of 42.1 percent and 47.4 percent, respectively. Coagulase-negative Staphylococcus was the most frequent germ. Intravitreal antibiotics plus remission for early flat vitrectomy was the most commonly employed therapeutic behavior. Conclusions: The incidence of post cataract surgery endophthalmitis in this series is in the range of that reported worldwide. It commonly presents acutely and with poor visual acuity. The most frequently isolated etiologic agent was coagulase-negative Staphylococcus(AU)

Background: Endophthalmitis is a serious infectious complication of cataract surgery, which may lead to vision loss. Aim: To evaluate the effectiveness of intracameral moxifloxacin in reducing endophthalmitis after cataract surgery in Chilean patients. Material and Methods: We reviewed all phacoemulsification surgeries performed between 2012 and 2020 at a public hospital. The use of intraoperative intracameral moxifloxacin and possible surgical complications were recorded. In patients with postoperative endophthalmitis, we reviewed their risk factors and clinical characteristics. Results: In the study, 22,869 phacoemulsification surgeries were registered, with an annual average of 2,541. The use of prophylactic intracameral moxifloxacin started progressively in 2014. In 2018 it was used in 88% of the surgeries. Fifteen eyes evolved with postoperative endophthalmitis, but none of these surgeries used intracameral moxifloxacin. Five and seven cases occurred in 2012 and 2013, respectively. There was a trend favoring moxifloxacin use, as a preventive measure for endophthalmitis, but the difference between groups was not significant (p = 0.56). In the group with endophthalmitis, 33.3% of the eyes were from patients with type 2 diabetes mellitus, in 13.3% there was rupture of the posterior capsule and 60% of the eyes corresponded to female patients. Since 2018 there is no record of endophthalmitis after cataract surgery performed in this center. Conclusions: Intracameral moxifloxacin showed a tendency to reduce the frequency of endophthalmitis after phacoemulsification surgery, but a longer observation period is required to reach statistical significance, due to the low frequency of this complication.

Purpose: This study was conducted to describe and validate a novel inflammatory score (IS) system in the management of infectious endophthalmitis. Methods: This was a prospective comparative non?interventional observational study. The study included the patients with clinical signs and symptoms of acute post?cataract surgery endophthalmitis (surgery within 6 weeks) with visual acuity from 6/18 to light perception. IS was scored by the clinical picture at two?levels of four ocular tissues on a scale of 0 (normal) to 4 (severe). Four masked graders of different levels of experience evaluated slit?lamp photographs. The concordance correlation coefficient was assessed between the slit?lamp clinical grading and photographic grading. We measured the concordance correlation coefficient, Pearson’s correlation (indicating precision), and the bias correction factor (indicating the accuracy). Results: The study included 43 eyes of 43 patients. The concordance correlation coefficient was 0.99 (95% CI 0.995 to 0.998). Both Pearson’s correlation coefficient and the bias correction were 0.99. The interclass correlation coefficient (ICC) was measured. The intra?rater ICC was 0.833 with good agreement (95% CI, 0.711 to 0.906; P < 0.001). Inter?rater ICC for consistency was 0.92 (95% CI 0.87 to 0.95). Inter?rater ICC for absolute agreement was 0.86 (95% CI 0.66 to 0.93). Conclusion: Currently used IS scoring in the study is a reliable, reproducible, and easy?to?apply scale to measure inflammation severity in endophthalmitis. We propose that it can have applications in decision?making for primary treatment and monitoring progression in acute infectious endophthalmitis

ABSTRACT Purpose: The study aimed to assess the anatomical and functional success rates of penetrating keratoplasty with temporary keratoprosthesis-assisted vitreoretinal surgery. Methods: This retrospective study included 15 eyes of 14 patients, recording demographic characteristics, pre-operative anterior and posterior segment pathologies, intraoperative complications, postoperative graft status, retinal attachment, and complications and evaluating anatomical and functional success rates. Results: The mean follow-up period was 29.8 ± 19.1(6-60) months. The most common pre-operative corneal pathology was graft abscess (7 eyes [46.7%]), and the most common diagnosis of the posterior segment was endophthalmitis (7 eyes [46.7%]). Five (33.3%) cases had visual acuity between 0.001-0.08. Pre-operative endophthalmitis was diagnosed in all five cases with anatomical failure.Conclusion: Temporary keratoprosthesis-assisted vitreoretinal surgery with penetrating keratoplasty is an effective method to treat acute/subacute pathologies of the concomitant anterior and posterior segment. However, results may vary on a case-by-case basis. Pre-operative endophthalmitis is a poor prognostic factor for long-term success.


RESUMO Objetivo: Avaliar as taxas de sucesso anatômico e funcional da ceratoplastia penetrante associada à cirurgia vitreorretiniana assistida por ceratoprótese temporária. Métodos: Foram incluídos neste estudo retrospectivo 15 olhos de 14 pacientes. Registraram-se as características demográficas, as patologias pré-operatórias dos segmentos anteriores e posteriores, as complicações perioperatórias, a condição pós-operatória do implante e a fixação e as complicações da retina. Foram avaliadas as taxas de sucesso anatômico e funcional. Resultados: O período médio de acompanhamento foi de 29,8 ± 19,1(6-60) meses. A patologia corneana pós-operatória mais comum foi o abscesso do implante (7 olhos, 46,7%) e o diagnóstico mais comum no segmento posterior foi a endoftalmite (7 olhos, 46,7%). Cinco casos (33,3%) mostraram acuidade visual entre 0,001 e 0,08. Foi diagnosticada endoftalmite pré-operatória em todos os 5 casos com insucesso anatômico. Conclusão: A cirurgia vitreorretiniana assistida por ceratoprótese temporária associada à ceratoplastia penetrante é um método eficaz de tratamento de patologias agudas e subagudas concomitantes nos segmentos anterior e posterior. Porém, os resultados podem variar de caso a caso. A endoftalmite pré-operatória é um fator de pior prognóstico de sucesso de longo prazo.

Purpose: To evaluate the clinical characteristics and visual acuity outcomes of patients who presented with endophthalmitis prior to and during the coronavirus disease 2019 (COVID-19) pandemic. Methods: This multicenter retrospective case series with historical controls included consecutive patients presenting with any form of endophthalmitis from March 1, 2019 to September 1, 2019 (pre-COVID-19) and from March 1, 2020 to September 1, 2020 (COVID-19) at Mayo Clinic Rochester (MCR), Health System (MCHS), Arizona (MCA), and Florida (MCF) sites. Cases were divided into "pre-COVID-19" versus "COVID-19" groups depending on when they first presented with endophthalmitis. Results: Twenty-eight cases of endophthalmitis presented to all Mayo Clinic sites during the study period. Of these, 10 patients presented during the first six months of the COVID 19 pandemic. During the same six-month period the year prior, 18 patients presented with endophthalmitis. Endophthalmitis etiology (post-injection, post-cataract extraction, post glaucoma filtering surgery, post-pars plana vitrectomy, endogenous, and others) was similar between both groups (P = 0.34), as was post-injection endophthalmitis rate (P = 0.69), days to presentation (P = 0.07), initial management (P = 0.11), culture-positivity rate (P = 0.70), and need for subsequent pars plana vitrectomy (P = 1). Visual acuity outcomes were similar between both groups at six months, however, the mean LogMAR visual acuity at presentation was worse in the COVID-19 group compared to the pre-COVID-19 group (2.44 vs 1.82; P = 0.026). Conclusion: Clinical characteristics and the post-injection endophthalmitis rate were similar during both periods, however, patients presented with worse vision during the pandemic suggesting that the pandemic may have contributed to delayed presentation, regardless, outcomes are still poor

ABSTRACT

Despite the introduction of novel sutureless posterior chamber intraocular lens (IOL) fixation techniques, some conditions still require suture-assisted scleral fixation. If the scleral fixation suture knot is left directly under the conjunctiva, it may become exposed, resulting in an increased risk of endophthalmitis. To avoid this problem, we offer a new alternative, simple, and safe way for burying the end of the suture using knots in this report.

AIM: To analyze the clinical characteristics and antibiotic sensitivity of traumatic endophthalmitis caused by Bacillus cereus and discuss the early diagnosis and treatment measures.METHODS: The data of 15 patients(15 eyes)with Bacillus cereus endophthalmitis admitted to the Eye Trauma Center of Shaanxi Provincial Eye Hospital from January 2019 to December 2021 were collected. The injury time, injury condition, preoperative visual acuity, corrected visual acuity in the last follow-up, operation method, bacterial culture and drug sensitivity test results were recorded.RESULTS: Among the 15 patients(15 eyes), 5 eyes with simple corneal perforating wound were treated with wound debridement and suture combined with intraocular antibiotic injection. And another 10 eyes with penetrating injuries combined with traumatic cataract and intraocular foreign bodies were treated with debridement, cataract extraction, removal of intraocular foreign bodies, vitrectomy combined with silicone oil tamponade and intraocular antibiotic injection. Among the 15 patients, 11 patients(73%)were treated effectively and the eyeball was preserved, and 4 patients(27%)were treated ineffectively and ocular evisceration was performed. Among the 11 patients with eyeball retention, 1(9%)had decreased vision, 1(9%)had unchanged vision, 4(36%)had improved vision by 1 level, and 5(45%)had improved vision by 2 levels, and the postoperative visual acuity was significantly improved. Correlation analysis showed that the course of disease was negatively correlated with corrected visual acuity(rs=-0.762, P=0.001). The cultures of vitreous humor and aqueous humor samples of patients were all grown in Bacillus cereus, and susceptibility tests were sensitive to vancomycin.CONCLUSIONS: Vancomycin is an effective drug for the treatment of Bacillus cereus endophthalmitis. Vitrectomy combined with silicone oil tamponade and intravitreal injection of vancomycin are effective method for the treatment of Bacillus cereus endophthalmitis.

ABSTRACT

Purpose: To assess the fluoroquinolone resistance pattern and trends among bacterial isolates from ocular infections over a 16?year period and explore alternative antibiotics in fluoroquinolone?resistant strains. Methods: In this retrospective, longitudinal study, the microbiology laboratory records of patients with different ocular infections diagnosed at an eye institute in central India from 2005–2020 were reviewed to determine the pattern of fluoroquinolone (ciprofloxacin, ofloxacin, gatifloxacin, and moxifloxacin) resistance. Antibiotic susceptibility testing was done using the Kirby–Bauer disc diffusion method. Results: In 725 Gram?positive bacteria, the resistance of ciprofloxacin, ofloxacin, gatifloxacin, and moxifloxacin was 55.9% (95% confidence interval [CI]: 52.2 – 59.6), 42.7% (95% CI: 39.0 – 46.4), 47.6% (95% CI: 43.9 – 51.3), and 45.6% (95% CI: 41.7–49.5), respectively. In 266 Gram?negative bacteria, the resistance of ciprofloxacin, ofloxacin, gatifloxacin, and moxifloxacin was 57.9% (95% CI: 51.9 – 63.9), 56.0% (95% CI: 49.7 – 62.1), 59.9% (95% CI: 53.8 – 66.0), and 74.3% (95% CI: 68.3 – 80.2), respectively. A declining trend in resistance to ciprofloxacin (P < 0.001), ofloxacin (P < 0.001), and moxifloxacin (P < 0.001) was seen in Gram?positive bacteria, whereas a reduction in resistance to only moxifloxacin (P = 0.04) was seen in Gram?negative bacteria. In fluoroquinolone?resistant Gram?positive bacteria, cefuroxime exhibited the highest susceptibility, whereas in fluoroquinolone?resistant Gram?negative bacteria, colistin exhibited the highest susceptibility. Conclusion: Fluoroquinolone resistance was high among bacteria from ocular infections in central India, but a declining trend in resistance to some of the fluoroquinolones was observed in recent times. Cefuroxime and colistin emerged as alternatives in fluoroquinolone?resistant Gram?positive and Gram?negative bacterial infections, respectively.

ABSTRACT Purpose: To analyze the epidemiological profiles of evisceration and enucleation cases in the ophthalmologic emergency department of a Brazilian tertiary hospital. Methods: Patients treated in the ophthalmologic emergency department of Hospital São Paulo (Universidade Federal de São Paulo) during the period 2013 to 2018 were retrospectively evaluated. Urgent cases of evisceration or enucleation surgery were included, and elective cases were excluded. The following information was extracted from the patients' medical records: demographic data, immediate and associated reasons for the surgical procedure, informed visual acuity, symptom duration until ophthalmologic care, complications, distance from the residence to the tertiary hospital, and time of hospitalization. Results: In total, 61 enucleations and 121 eviscerations were included in this study. The patients had a mean age of 63.27 ± 18.68 years. Of the patients, 99 were male (54.40%), and 83 were female (45.60%). The indications for evisceration or enucleation were corneal perforation with (44.50%) and without (23.63%) signs of infection, endophthalmitis (15.38%), ocular trauma (14.29%), neoplasia (0.55%), burn accident (1.10%), and phthisis bulbi (0.55%). The self-reported visual acuity was no light perception (87.36%) or light perception (1.10%). However, 3.30% of the patients did not cooperate with the examination, and no information on visual acuity was available for the remaining 8.24%. The mean symptom duration before ophthalmologic care was sought was 18.32 days. Two patients had sympathetic ophthalmia after evisceration. Conclusions: More eviscerations than enucleations were performed throughout the study period. The most common demographic characteristics were age >60 years and male sex. The main indications for urgent evisceration and enucleation procedures were corneal perforation with and without infection, endophthalmitis, and ocular trauma. The findings from this study could guide clinicians in performing preventive measures to avoid destructive eye procedures.


RESUMO Objetivo: Analisar o perfil epidemiológico dos casos de evisceração e enucleação no pronto-socorro oftalmológico de um hospital terciário brasileiro. Métodos: Análise retrospectiva dos casos tratados no pronto-socorro oftalmológico do Hospital São Paulo (Universidade Federal de São Paulo) entre os anos de 2013 a 2018. Os casos urgentes de evisceração e enucleação foram incluídos e os casos eletivos foram excluídos. A análise dos prontuários médicos foi baseada em: dados demográficos, causas imediatas e associadas ao procedimento, acuidade visual informada, duração dos sintomas antes do atendimento oftalmológico, complicações, distância da residência até o hospital e tempo de hospitalização. Resultados: 61 enucleações e 121 eviscerações foram incluídas no estudo. Os pacientes tinham uma média de idade de 63,27 ± 18,68 anos; 99 eram do sexo masculino (54,50%) e 83 do sexo feminino (45,60%). As indicações de evisceração e enucleação foram: perfuração corneana com (44,50%) e sem (23,63%) sinais infecciosos, endoftalmite (15,38%), trauma ocular (14,29%), neoplasia (0,55%), queimadura (1,10%) e phthisis bulbi (0,55%). A acuidade visual informada foi de ausência de percepção luminosa (87,36%), percepção luminosa (1.10%), ausência de colaboração (3,30%) e sem dados informados (8,24%). A média de tempo até a busca pelo serviço oftalmológico foi de 18,32 dias. Houve 2 casos de oftalmia simpática após evisceração. Conclusões: Eviscerações foram predominantemente realizadas em comparação a enucleações em todo o período de estudo. As características demográficas mais comuns foram idade >60 anos e sexo masculino. As principais indicações para procedimentos urgentes de evisceração e enucleação foram perfuração corneana com e sem infecção, endoftalmite e trauma ocular. Este estudo poderia guiar medidas preventivas para evitar procedimentos oculares destrutivos.

Objetivo: Determinar la incidencia de infecciones relacionadas a la bula de filtración en pacientes sometidos a trabeculectomía; así como su comportamiento clínico. Métodos: Se realizó un estudio descriptivo longitudinal y retrospectivo. El universo estuvo comprendido por 1320 ojos con trabeculectomía y muestra conformada por 7 ojos con diagnóstico de infección relacionada a la bula de filtración operados en el Centro Oftalmológico de Villa Clara desde enero del 2012 hasta diciembre del 2021. Resultados: La incidencia global de infección en la bula de filtración fue de 0,53 por ciento. La incidencia de blebitis y de blebitis-endoftalmitis fue 0,15 por ciento y 0,38 por ciento, respectivamente. Los hombres fueron los más afectados y la edad media fue de 64,7 años. En todos los casos, la infección se presentó de forma tardía con una media de 4,1 años. El 57,1 por ciento tuvo una agudeza visual mejor corregida al inicio de la infección de movimiento de manos. Se reportó crecimiento bacteriano en un 57,1 por ciento, el estafilococo coagulasa negativo fue el germen más frecuente. El control de la infección se logró en 6 pacientes mientras 2/3 de ellos no tuvo mejoría de la agudeza visual mejor corregida a la resolución de la infección, todos con blebitis-endoftalmitis. Conclusiones: Las infecciones relacionadas a la bula de filtración son poco comunes. Se presentan con frecuencia años después de la cirugía filtrante y con mala agudeza visual. Los ojos con blebitis-endoftalmitis tienen una pobre recuperación visual a pesar del tratamiento(AU)


Purpose: To determine the incidence of filtration bullae-related infections in patients undergoing trabeculectomy; as well as their clinical behavior. Methods: A longitudinal and retrospective descriptive study was performed. The universe was comprised of 1320 eyes with trabeculectomy and the sample consisted of 7 eyes with a diagnosis of infection related to the filtration bulla operated at the Villa Clara Ophthalmologic Center between January 2012 and December 2021. Results: The overall incidence of infection in the filtration bullae was 0.53 percent. The incidence of blebitis and blebitis-endophthalmitis was 0.15 percent and 0.38 percent, respectively. Males were the most affected and the average age was 64.7 years. In all cases, the infection presented late with a average age of 4.1 years. Fifty-seven point one percent had best corrected visual acuity at the onset of hand motion infection. Bacterial growth was reported in 57.1 percent, coagulase negative staphylococcus was the most frequent germ. Infection control was achieved in 6 patients while 2/3 of them had no improvement of best corrected visual acuity at resolution of infection, all with blebitis-endophthalmitis. Conclusions: Infections related to filtration bullae are uncommon. They occur frequently years after filtering surgery and with poor visual acuity. Eyes with blebitis-endophthalmitis have poor visual recovery despite treatment(AU)

Abstract Objective: Acknowledging the rates of endophthalmitis after cataract surgery is very important to understand the seriousness of the disease. Until now, there is no data of this situation in Argentina. The aim of this study was to estimate the incidence of postoperative endophthalmitis after cataract surgery at a tertiary hospital, a medical facility which provides a high degree of subspecialty expertise, in Buenos Aires, Argentina. Method: Retrospective cohort study of adult patients who underwent cataract phacoemulsification sur gery between 2006 and 2020 at Hospital Italiano de Buenos Aires, was performed. Patients with endophthalmitis diagnosis within the first 6 weeks after surgery were included. Those who underwent extracapsular surgery or combined surgeries were excluded. Results: A total of 29 326 cataract surgeries were performed in ten years. The annual incidence of acute postoperative endophthalmitis after cataract surgery was 0.102% (95% CI 0.069-0.146). Thirty cases of endophthalmitis were diagnosed. The main microorganism was Pseudomonas aeruginosa (45.4%). Final visual acuity was higher than 0.3 (LogMAR) in 46.6% of the patients. Conclusion: The incidence of endophthalmitis after cataract surgery in this hospital is within the range reported globally. Knowing the regional incidence contributes to decide if new prophylaxis measures are necessary.


Resumen Objetivo: Conocer las tasas de endoftalmitis después de la cirugía de cataratas es muy importante para comprender la gravedad de la enfermedad. Hasta el momento, no hay datos de su situación en Argentina. El objetivo de este estudio fue estimar la incidencia de endoftalmitis postoperatoria tras cirugía de catarata en un hospital de tercer nivel o alta complejidad de Buenos Aires, Argentina. Método: Se realizó un estudio de cohorte retrospectivo de pacientes adultos operados de facoemulsificación de cataratas entre 2006 y 2020 en el Hospital Italiano de Buenos Aires. Se incluyeron pacientes con diagnóstico de endoftalmitis dentro de las prime ras 6 semanas posteriores a la cirugía. Se excluyeron los pacientes que se sometieron a cirugía extracapsular o cirugías combinadas. Resultados: En diez años se realizaron un total de 29.326 operaciones de cataratas. La incidencia anual de endoftalmitis postoperatoria aguda después de la cirugía de cataratas fue del 0.102% (IC del 95%: 0.069-0.146). Treinta casos de endoftalmitis fueron identificados. El principal microorganismo fue Pseudomonas aeruginosa (45.4%). La agudeza visual final fue superior a 0.3 (LogMAR) en el 46.6% de los pacientes. Conclusión: La incidencia de endoftalmitis después de la cirugía de cataratas en este hospital está dentro del rango reportado a nivel mundial. Conocer la incidencia regional contribuye a decidir si son necesarias nuevas medidas de profilaxis.

Abstract Scedosporium/Lomentospora species are widely distributed in nature. They are gen-erally saprophytes, but can cause opportunistic infections in immunocompromised patients and occasionally in immunocompetent patients that are difficult to treat due to high lev-els of antifungal resistance. The distribution of Scedosporium/Lomentospora species shows regional differences. Scedosporium boydii and Scedosporium apiospermum are the most fre-quently isolated species in our region, whereas Scedosporium aurantiacum is more common in other regions. We describe the first isolation in Argentina of S. aurantiacum in a vitreous humor infection from a previously healthy patient after traumatic injury in her left eye. Due to the suspicion of fungal endophthalmitis, a mycological study of the vitreous humor was performed. The culture allowed the isolation of S. aurantiacum. The patient was treated with voriconazole with favorable clinic evolution.


Resumen Las especies de Scedosporium/Lamentospora se encuentran ampliamente distribuidas en la naturaleza. En general son saprofitas, pero pueden causar infecciones oportunistas de difícil tratamiento debido a sus altos niveles de resistencia a los antifúngicos en individuos inmunocomprometidos y, ocasionalmente, en personas inmunocompetentes. La distribución de las especies de Scedosporium/Lamentospora muestra diferencias regionales. Scedosporium boydii y S. apiospermum son las especies más frecuentemente aisladas en nuestra región, mientras que en otras S. aurantiacum es más común. Presentamos el primer aislamiento en Argentina de S. aurantiacum de una infección de humor vítreo de un paciente previamente sano que sufrió una lesión traumática. El paciente fue tratado con voriconazol y tuvo una evolución clínica favorable.

Purpose: The aim of this study was to present the signs, symptoms, management, and outcome of a series of cases of cluster endophthalmitis caused by a multi-drug resistant fungus, Trichosporon. Methods: This was a retrospective, non-randomized, consecutive interventional case series. Ten cases of postoperative endophthalmitis operated by a surgeon on three consecutive operation theater (OT) days presented 3�months after their surgery. All cases were microbiologically confirmed. The pathogen was found to be resistant to most antifungals, including amphotericin B. The cases had a latent period of around 45 days. Management of endophthalmitis included intravitreal injections, anterior chamber (AC) lavage, Pars Plana vitrectomy (PPV), posterior capsulotomy, IOL, and capsular bag removal. Multiple intravitreal injections were required due to recurrence of infections after initial improvement with voriconazole injections. Results: Structural integrity was maintained and infection-free status was achieved in all the eyes. The presenting vision ranged from 6/60 to PL (perception of light). Seven out of 10 had improvement in their final vision over the presenting vision. Final outcome of four patients had vision of 6/24 or better, 4 patients had vision in the range of 2/60 to 6/36 and 2 patients had PL. Conclusion: Trichosporon can cause devasting infections even in the immunocompetent, especially in association with implants and catheters. Triazoles form the mainstay of treatment of Trichosporon infection due to the high susceptibility of the organism in vitro. A regimen including voriconazole and amphotericin B may prove to be the most effective. This is the first report of an outbreak of cluster endophthalmitis caused by Trichosporon

Purpose: To compare different endophthalmitis prophylaxis methods adopted in cataract surgery (manual small-incision cataract surgery and phacoemulsification) between the years 2013 and 2021 in the community eye care section of a tertiary eye care hospital in South India and report their outcomes. Methods: All cataract surgeries performed from January 2013 to December 2021 (2,46,874 surgeries) at a single center were included in this retrospective study. The different endophthalmitis rates with each regimen were analyzed and evaluated. Results: 70,081 surgeries were performed from January 2013 to February 2015, where Tobramycin was added to Balanced Salt Solution (BSS) (Group A). From March 2015 to January 2017, 63,245 surgeries were performed when intracameral Moxifloxacin was given (Group B). From February 2017 to December 2021, 1,13,548 surgeries were performed were Amikacin was added to BSS (Group C). In total, 42 cases of postoperative endophthalmitis were reported during the study period (0.02%). There was no significant difference in the endophthalmitis rates between groups A and B (P = 0.4152); however, there was a significant decrease in endophthalmitis rates in group C when compared with group A (P = 0.04) and group B (P = 0.006). Conclusion: There was a significant reduction in the rates of endophthalmitis following the addition of amikacin in irrigating BSS. Nocardia was one of the predominant organisms isolated from these endophthalmitis patients. This is the first single-center study to report a comparative analysis of different endophthalmitis prophylactic measures in a community eye care set up with a high incidence of Nocardia endophthalmitis prevented with amikacin in BSS irrigating solution.
